I voted for three, and I'm not surprised to see that it's winning. I sort of expected it. Personally, I'd go for the high end of three. I might also include in three some limited neuromuscular damage from racking, strappado, or torture with ropes. She should be in pain long afterwards. Also possible pulling of the fingernails/toenails and drilling of the teeth. (Which is fixable by cosmetic dentistry, so it's arguably not permanent scarring.)

Excision of the clit and/or labia is pretty extreme, but it's a definite maybe.  It's symbolic.
